Special cleanup request form

• You must
  • be the utility bill payer to make this request.
  • include a copy of your valid state-issued ID to verify you're the utility bill payer.
  • submit the application at least 5 business days before the requested cleanup date.
• We'll send a foreman to verify we can collect all items set out.
  • If approved the cleanup will take place on or after the requested cleanup date.
• We'll add the fee for the special cleanup to your utility bill.
